Why Eye Services Remain a Top OIG Audit Priority

Eye and vision services continue to draw close attention from the Office of Inspector General—and the trend isn’t slowing down. 

Each year, Medicare and Medicaid spend billions on eye-related care, making these services a consistent audit focus. Recent OIG audits highlight recurring issues tied to Evaluation and Management coding, modifier use, and diagnostic imaging—areas that often feel routine but carry significant compliance risk. 

Audits have flagged concerns around E/M codes like 99309 and 99310, the use of modifier -25 with office and outpatient visits, and improper application of modifiers -59 and -XU. Imaging services such as SCODI (CPT® 92132–92134) and fundus photography (CPT® 92250) have also appeared repeatedly in audit findings.
