Decoding A "Detailed" Examination for E/M Services

It can be a total nightmare to figure out the definition of a “detailed” examination using 1995 Documentation Guidelines for Evaluation and Management Services. It doesn’t help that CMS has been reluctant to expand the definition. It’s frustrating, isn’t it?

Luckily, some of the MACs have provided additional instructions/clarifications that we can use to improve our understanding. With the additional clarifications we can learn how to differentiate between “body areas” and “organ systems,” how to audit the examination portion of an E/M service observing MAC’s guidance with 1995 DGs and how to understand when (if) the 1997 criteria may better fit your practice.
