While their training includes how to document treatment, that training could be considered minimal, at best. Given that poor documentation standards could result in patient mismanagement, and a loss of revenue, this gap in training is of great concern.

Of course, because documentation is also used by health plans to process reimbursement payments, and to assess the overall risk of their beneficiaries (or as we call it, Risk Adjustment), the stakes are even higher.
